Forms for Use of Vertebrate Animals in Research or Teaching

Tips and Instructions:

• All new submissions require the following forms:
  • Main Submission Form
  • PI Assurance Form
    
• Appendices may or may not be required, depending on the project
  • Download and complete only those appendices that apply and include them with your submission.
    
• Attach all applicable review documents to an email as WORD documents and email to irb-iacuc@radford.edu.
  
  
• The PI Assurance Form is to be print, signed by the PI and the appropriate departmental representative, and forward the original to the IACUC Office. The approval letter will not be released until this form is on file at the IACUC Office.

Please allow at least 4-6 weeks for protocol review, depending on the complexity of the project and the extent of revisions required.
