What is an IFSP?

An Individualized Family Service Plan (IFSP) is a written plan of action for your child and family. The IFSP will bring together the strengths and needs of your child and build upon the goals and priorities you have for your child and family. You and your family are vital participants in the development of this plan. The plan is a good starting point for deciding what activities and services are needed to help your child and family reach your goals. The IFSP is reviewed at least every six months and revised each year. Changes in services cannot be made without Prior Written Notice to the parents after which an IFSP meeting is held.
