These CoPs are focused on sharing information relative to secondary transition planning, including but not limited to:  age-appropriate transition assessment, community resources, postsecondary goals, family engagement, and school/community successes and challenges.

If you are involved with any aspect of postsecondary transition support in a region 6 or 7 school/division and would like to participate in these CoPs, please contact Katie Ginn at kginn@radford.edu.

Secondary Transition Planning Resources

CTI-Logo

The Virginia Department of Education has created a centralized statewide portal for information, resources, demonstration, and research for educators and other stakeholders in the transition of youth with disabilities. The Center on Transition Innovations (CTI) at Virginia Commonwealth University provides evidence-based resources and information along with emerging practices in the field. The results of research studies and demonstration projects conducted here in Virginia help us shape the ongoing work of CTI.
