This page is the hub for RUC students, faculty and staff to report any concerns. If you see or hear something that gives you cause for concern, please submit any information through the appropriate form.

Reporting Forms

Behavior of Concern Form
The Behavioral Consultation Team accepts reports from any person who is concerned about the health, safety or welfare of a Radford student or employee. For more information about the BCP team, visit the Behavioral Consultation Team page.

Conduct Incident Form
This form should be used to report student behavior that is disruptive, violates the Standards of Student Conduct, violates any university policy or that infringes on a person’s ability to pursue their academic work. For more information about Student Conduct, visit the Conduct and Professionalism page.  

Academic Integrity Form
This form should be used to report any alleged academic integrity violations as outlined in the Student Standards of Conduct. Faculty members who suspect academic integrity violations should contact RUC Student Affairs at 540-831-1691 or RUCCampusLife@radford.edu for consultation.

SAVES Anonymous Reporting Form
This form is to report a sexual misconduct to the Radford University Substance Abuse and Violence Education Support (SAVES) Office. Reporting sexual misconducts assists with the completion of statistical records for assaults that occur to Radford University students, on- and off-campus. Filing this form will not result in an investigation, but is intended to convey needed information to assist the university in addressing the issues of sexual assault. Information contained on the form will remain confidential and will only be used in non-identifying ways. This is a non-confidential online report for students, faculty, staff, contractors, and visitors. The information provided will not be disseminated to the public unless required by the law.

Mandatory Reporting Form for Title IX Incidents from Employees
This is a non-confidential incident report for employees, including student employees. All Radford University employees in receipt of a complaint or report must report it without delay. The information will not be disseminated to the public unless required by law.

This is a non-confidential incident report for employees, including student employees. All Radford University employees in receipt of a complaint or report must report it without delay.
