NURS 725: Advanced Family Nursing: Chronic Illness Across the Lifespan-Clinical      

Prerequisite:  NURS 710, NURS 712, NURS 713, NURS 716 and concurrent with NURS 724; or permission of faculty

This course is the clinical practicum for NURS 724, Advanced Family Nursing: Chronic Illness Across the Lifespan.

Detailed Description of Content of the Course

This clinical course will provide a minimum of 90 hours of clinical practice experience with community preceptors including but not limited to NPs, MDs, DOs, and PAs practicing in the community with the focus of chronic illness in primary care across the lifespan.      

Detailed Description of Conduct of Course

The non-clinical portions of the course will be online to facilitate communication between faculty & students and may incorporate faculty/student directed discussions, reading assignments, and written work which will include clinical logs. Because the course is online, audio or video presentations/lectures, discussion boards, chat rooms and other instructional technology strategies may be used for communication purposes. In addition it may include but not limited to:  video demonstration(s) with consenting patients of various components demonstrating appropriate clinical decision making of the evaluation and management of patient care across the lifespan.

Goals and Objectives of this Course

At the conclusion of the course the student will be able to:

1. Provide primary care to individuals, families, and groups with chronic illness.

2. Employ effective communication (oral and written) and collaborative skills within the framework of primary care (with both colleagues and patients

3. Demonstrate appropriate consultative and referral skills for chronic illness management across the lifespan in primary care.

4. Develop, implement, and evaluate interventions to address health promotion/disease prevention in families across the lifespan utilizing concepts of psychosocial dimensions and cultural diversity.

5. Conduct a comprehensive and systematic assessment of health and illness parameters in complex clinical situations across the lifespan.

6. Design, implement, and evaluate therapeutic interventions (education, medication choice, referral and follow-up) based on nursing and other sciences

7.  Develop and sustain therapeutic relationships and partnerships with patients across the lifespan and other professionals to facilitate optimal care and patient outcomes

8. Demonstrate advanced levels of clinical judgment, systems thinking and accountability in designing, delivering, and evaluating evidenced-based care to improve patient outcomes.

9. Educate and guide families across the lifespan through complex health and situational transitions

10. Demonstrate practice acumen for clinical decision making

11. Maintain appropriate clinical records and documentation.

Assessment Measures

May include but are not limited to:  Preceptor and faculty evaluation of clinical performance, completion of clinical logs, case studies; quizzes, and student presentations, and other strategies including using available technology for direct observation of clinical practice.
