Spring 2021 RUC Planning and Required Travel Declaration Form

As we conclude the Fall 2020 semester, all students are encouraged to have an enjoyable and healthy break. During the semester break, Radford University officials will be working diligently to fully prepare for your safe return.

In order to ensure that the University takes all necessary precautions to keep you and your fellow students healthy, all students are being required to complete a Travel Declaration Form. The survey asks a series of questions based on your travel plans and related activities between December 30, 2020 and your return to RUC, the City of Roanoke, and/or the Roanoke Valley. The survey is available at https://radford.co1.qualtrics.com/jfe/form/SV_9GP7i5d39WQvekt and must be completed by all students no later than January 3, 2021.

In order to complete the survey, you must be logged into the OneCampus portal. If you are not already logged into the system, then you will need to enter your user name and password to access and submit the survey.

Multiple submissions will be accepted, and your most recent submission will serve as your official response. Therefore, you are encouraged to submit this information now based on your planned activities, and then resubmit again if your plans change.

Survey responses will be utilized to inform COVID-19 prevalence testing to be held in mid-January. RUC testing will be held on January 15, 2021 through January 16, 2021, with the first day of classes on January 19, 2021. Specific information for students required to participate in prevalence testing will be distributed to identified individuals by January 8, 2021.

For students living in University operated housing, you will receive information related to returning to campus for the Spring 2021 semester by December 15, 2020, including a method to sign-up for an appointment to return to your campus housing assignment in The Patrick Henry, if applicable. Communications regarding your campus return and any required testing will be distributed by departments or offices under the Division of Student Affairs.
