Addendum for All Vendor Contracts
Addendum must be used with all contracts supplied by vendor.
Catering Request/Food Services Order Form
This form is used for any food purchase or catering request.
Office Supplies Order Form
This form must accompany any purchase request from The Supply Room Companies.
Printing Services Photocopy Request Form
This form is used for RU services printing request.
Purchase Order/Payment Request Form
This form is used to request a purchase/payment from an RU budget.
Request for Travel Form
Fill out this form completely when travel is required.
This form must be approved and returned to the account manager prior to reserving accommodations, registrations, vehicles, etc.
RU Vendor Contract Form
This form is used when vendors do not have their own contracts.
SFC Base Budget Request Forms
SFC Special Initiative Form
These forms are used by departments and student organizations that request their annual budget from SFC..
Taxpayer Identification Form
This form is used to collect federal taxpayer information from individuals/businesses.
Travel Guideline Tables
This form outlines approved state rates for various travel needs in-state and out-of-state.
(i.e. meal per diems, motel rates, etc.)
Travel Reimbursement Form
This form is used to record expenses after travel is complete.
Vehicle Rental Request Form
This form is used for Enterprise or Facilities management vehicle rentals.

Applications are due to Hurlburt Student Center Room 235 by 4:00 pm on Wednesdays to be reviewed at the following week's CPC meeting. CPC applications must be submitted at least 4 weeks prior to the date of the on-campus event. Applications must send a representative to the CPC meeting at which their application will be reviewed.

Travel Funding Request &
CPC Travel Funding Application
Applications are due to Hurlburt Student Center Room 235 by 4:00 pm on Wednesdays to be reviewed at the following week's CPC meeting. CPC applications must be submitted at least 5 weeks prior to the first date of travel domestically and 8 weeks prior to international travel. Applications must send a representative to the CPC meeting at which their application will be reviewed.

Nominations for RU's Outstanding Student Award and Who's Who should be submitted to Box 6895 or Hurlburt Student Center Room 235 by 12:00 pm on Friday, March 5, 2010.
The Student Awards Committee is currently accepting nominations for awards presented annually at the Student Awards Ceremony and Reception. All applications for awards are due on Monday, March 22, 2010. Recipients will be selected by the Student Awards Committee.
