Appeal Forms

Students may download appeal forms by clicking on the links below.

 

Sanction Appeal Form -  If you accepted responsibility for charges and would like to appeal your sanctions, the appeal form and written statement must be submitted to the Office of the Dean of Students or the Office of Residential Life as appropriate within 48 hours or by 9:00 a.m. on the next university business day if the deadline falls on a weekend or after 5:00 p.m. on a weekday.

 

Hearing Appeal Form - If you went to a hearing and would like to appeal the outcome of that hearing, the appeal form and written statement  must be submitted to the Office of the Dean of Students within 72 hours of receipt of the original written decision, or by 9:00 a.m. on the next university business day if the deadline falls on a weekend or after 5:00 p.m. on a weekday.
