Graduation Application -- This form must be completed and turned in to your faculty advisor. Please read and follow the directions on the second page.
Declaration of Intent to Participate – This form is to be used if you plan on participating in graduation and will be able to complete your degree requirements during the following semester (participate in May and complete requirements in Summer, participate in December and complete requirements in Spring).
