Application for Graduation and check-out checklist

Declaration of Intent to Participate in Commencement Program
    This form is used if you wish to participate in the commencement 
    ceremony, but have not yet completed the requirements for
    graduation.  For example, you wish to walk in the Spring 
    Commencement Ceremony, but you have summer classes to complete
    before you graduate.
