On the first day of employment, you may give employees the packet of information and have them complete the forms (Items 3-8), or bring the employees to the Department of Human Resources (704 Clement Street).
Please note forms marked with "*" are required to be completed within 3 days of employment, or employment will be terminated. All forms need to be returned to Department of Human Resources at Box 6889. Should you have any questions, please do not hesitate to call our office (540-831-5584).
For compliance with the Federal Law, the Department of Human Resources needs a copy of the employee's social security card. This is in addition to the required I-9, VA-4, and W-4 documents necessary to establish the employee on payroll.
