STATE VEHICLE ACCIDENT-REPORTING REQUIREMENTS
If you are involved in an accident while operating a state vehicle or a rental vehicle used for business purposes:
!. Stop at the scene of the crash or as close to the scene as possible without blocking traffic. You may move the vehicles before the police arrive if the vehicles create a traffic hazard. Take all necessary precautions to prevent further accidents at the scene
2. Give any help you can if someone is injured. Do not attempt to move an injured person from a wrecked vehicle unless you have the necessary medical training or there is an immediate danger such as fire.
3. Report the crash to the local police as quickly as possible. If the accident occurred on campus, call the RU Police (831-5500) and the Safety Office ( 831-7790).
4. Exchange information with other people involved in the crash as soon as possible. Be sure to get the following information:
5. Fill out the "Statement of Accident" report form (located in the glove compartment). Have the driver of the other vehicle complete this form also. Return the completed form to the Safety Office at PO BOX 6909, Radford, Virginia, 24142.
6. Notify the State's Insurance Carrier (Crawford & Company) by calling their 24-hour toll-free number 1-866-219-6120. Ask for Wanda Brooks or Jay Bey. Identify yourself as a Commonwealth of Virginia, Radford University employee. If the other driver wishes to file a claim, have them call Crawford also.
7. If the vehicle was a rental, please notify the rental agency also. RU contracts with Enterprise. Talk to Kevin McBurnette at 757-597-2243.
8. Do not make a statement of any kind to anyone other than your employer, a law enforcement officer or a representative of the insurance or rental company.
9. Get a copy of the accident report from the police officer at the scene.
