Online Forms

The following is a listing, by department, of forms which are available online at RU. Some of these forms allow for electronic submission of information (noted as "E-form"), while others are available simply as html or pdf documents which can be printed from your computer, filled in and returned via mail, fax or by hand to the appropriate office. Some also are online as printable Word documents. A note after each listing shows in which format the form exists. (Note: In order to view pdf files, you must have ADOBE Acrobat Reader installed on your computer. The reader may be obtained from the ADOBE website free of charge.) Forms which are listed but which are not linked to an online version are "under construction" and will be available soon. Questions or problems concerning individual forms should be addressed to the department responsible for that form. Contact information is included with each department listing. Departments wanting to add a link to a new form which has been placed online should contact Rick Rogers at rrogers@radford.edu
