Federal Direct Loan Additional Loan Request Form

This is not an application for the Federal Direct Loan program. To apply for a Federal Direct Loan, you must complete the Free Application for Federal Student Aid (FAFSA) and complete the financial aid application process. This application is to be used by students who already have a Federal Direct Loan awarded to them and are requesting an additional loan amount due to the cancellation of aid, a PLUS denial, etc.

Borrower Certification: I request the Financial Aid Office forward my lender and guarantor the data required to process my Federal Direct Loan for the loan period/amount indicated above. I understand a Federal Direct Loan Master Promissory Note must be signed through the Dept. of Education website (if it has not been done). I request a Subsidized and /or Unsubsidized Federal Direct Loan in the amount indicated above not to exceed the maximum amount for which I am eligible to borrow. I understand Radford University will determine what portion of the loan is eligible for federal interest subsidies. I certify all information is true and correct.
